Overview

The Bridge to Wellness Integrated Care program began as an extension of the psychiatric rehabilitation programming because many of the people we serve lack access to critical, life-saving primary care due to a variety of barriers. Our goal is to eliminate those barriers, reduce hospitalizations, and save lives. We have two health professionals who will meet you at the location that is best for you. A licensed advanced practice nurse (APN), with more than 20 years of experience in community based primary care, and a bi-lingual licensed practical nurse (LPN) who specializes in wellness planning and activities.
